Page 1 The Latino Health Insurance Program (LHIP) is an intervention designed to respond to the high rate of un-insurance and underinsurance among minority residents in Massachusetts, such as Latinos, and to the multiple obstacles that impede them from applying, most notably lack of information, and working for employers, who do not sponsor health insurance. LHIP is a community-based program located in East Boston, MetroWest, Waltham and, Saugus, which focuses on educating minority families with the goal of increasing their access to medical services, self reliance and knowledge of the healthcare system. Our program provides advocacy and assistance to all Latinos in Massachusetts and other immigrant groups in need of medical care. In addition to our multiple services, which include assistance with insurance applications, we have additional benefits available to all families, individuals, small businesses, and community churches.As a member you are eligible to receive the following services: Assistance to apply for health insurance program subsidized by the state, such as MassHealth and not subsidized programs (private coverage).Assist members with early and frequent contact with MassHealth Act as family advocate by being liaison between Medicaid representatives and familiesAs member you are eligible to other benefits and discounts through our partners organizationsFollowed until successfully re-apply for coverageAssistance to apply for your renewalsReferrals to primary care providers and specialists for connecting families with care or through direct contact with hospitals and community health centersOther services provided like assistance to apply for SNAP (food stamps), and referrals to other collaborators and partners, such as free clinics, and attorneys. PCPs selections and appointmentsAssistance to apply for nutritional programsProvide Evidence-Based Healthy Aging Classes Workshops and education: obesity prevention program, cancer, diabetes and hypertensionPrograms for seniorsMembers will receive public education designed to address risk factors, self-management and reduce burden of diseasesMembers are linked to community resources, and groups with a focus on prevention and the determinants of healthRegular review of plan and proactive interventions to ensure meeting target Access to right services from coordination services supported by community partnersFollow-up reminders through telephone servicesRequisites to be a LHIP member:- Enjoy our mission and support our program.- Apply to medical coverage or service through our program.- Be more than 18 year of age. - Pay you membership on time (at least 30 days before the due day). - You will have an annual membership, and need to renew it before the expiration day. You will receive a correspondence 60 days prior to your due day. Please avoid cancelation.

Page 2 Milagros Abreu, MD, MPHMilagros Abreu is the Director and Founder of the Latino Health Insurance Program (LHIP); coordinates research efforts and surveillances with goals of improving health access and health care for minority residents in Massachusetts and US. Current research studies include increase access to preventive medicine, cross-culture medicine, workforce diversity and the impact of language on health care. The rate of uninsured Latino children and their families is more than twice that of the general population in Massachusetts an in the USA, despite the fact that all children are eligible for public health insurance. The LHIP is bringing several fractionated groups within the community together, to step up to the plate and work together to obtain health care for themselves and their families. With health insurance, children are more likely to get regular checkups, have fewer emergency medial visits and are less likely to be in poor health. LHIP is a community-based program located in MetroWest, East Boston , Waltham, Worcester, and Saugus, which focuses on educating minority families with the goal of increasing their access to medical services, self reliance and knowledge of the healthcare system. Due to the success of Dr. Abreu’s initiative, future programs are being planned to reach more towns in Massachusetts and immigrant communities nationwide.

The Latino Health Insurance Program, IncTo: Health care and dental providers, community churches, organizations, and, businesses in Massachusetts To whom it may concern,The goal of the Latino Health Insurance Program is to inform, educate, and to enroll minority residents from Massachusetts into medical, dental coverage and aditional health services (SNAP) in the state. As you know, Latinos represent the largest minority population in Massachusetts and in the USA, and it is consider one of the fastest growing populations in the country. Beside these facts, Latinos experience the lowest number of people enrolled into medical and dental coverage and other health services locally and nationwide. The mission of the Latino Health Insurance Program is to increase the number of Latinos and minority residents enrolled into medical coverage and per consequence increase their access to medical care. Objectives of the LHIP:-Invite community organizations, health care and dental providers to become members from our program in the state of Massachusetts and around the country.- Increase the number of Latinos enrolled into comprehensible and affordable medical and dental coverage in the USA.- Become an organization to advocate and promote equal access to affordable medical and dental care for immigrant populations in the USA.- Educate members to understand the benefits of preventable services and reduce their use of unnecessary emergency services.- Increase the number of Latinos to have a source of primary care in the USA. Activities:-Provide educational workshops about access to medical and dental care to Latinos in the USA.Provide Evidence-Based Healthy Aging Classes- Provide educational workshops on how to prevent chronic conditions, such as cardiovascular, diabetes, obesity, and cancer to participating members.-Enroll families into medical and dental coverage. -Serve as a support group to help minority residents on their medical needs.Support from participating members and organizationsInterested individuals who support our mission can provide a donation to become members.Our members annual fees help us to provide services to community residents, participating members, pay our case managers and their transportation to churches, and contribute with our program mission of increasing access to medical care for minority residents in Massachusetts.
