"Helping You Take Care of Yourself":

Cancer Prevention Series provided with material from the Massachusetts Department of Public Health. Classes offered for Breast, Cervical, Colorectal, and Prostate Cancer targeting 40-64 yr olds.

Chronic Disease Self Management Program/ Tomando Control de su Salud 

6 week curriculum addressing various topics about living with and taking control of a chronic illness. 

Diabetes Program 

The workshops are effective by the process and the mutual support from each participant. The classes include the techniques to deal with the symptoms of diabetes, fatigue and pain. The participants will meet once a week for six weeks for 2 1/2 hours.  ​​

Care Coordination Services

Community Health Education 

​​The prevention clinic helps people who have chronic medical conditions: such as diabetes, hypertension, cancer, arthritis, obesity, and others. The clinic also assists the elderly in preventing falls that could cause injuries. Our doctor refer patients to health education classes, such as CDSMP, Matter of Balance, Diabetes Self- Management, DPP, and Learning How to Take Care of Yourself to reduce risk of chronic conditions or complication of these diseases and improve health status.

Benefit Enrollment Center Services (BECs) 

Earlier this year, Latino Health Insurance Program (LHIP) began offering new services in the form of BECs. BECs helps elderly individuals, low-income residents, and those with disabilities in getting access to additional aid and benefits programs.
